CCMH Insurance / Financial Assistance Guide
The following resources are designed to assist your need for financial assistance with your care.
- Medicaid – Medicaid is a primary source of insurance for low-income and disadvantaged individuals. Medicaid applications are required for other forms of assistance. To complete an application, you can apply online at: www.healthcare.gov or contact the local Social Services office. Cavalier County’s number is (701) 256-2175. Eligibility is based on disadvantage or purely off of income.
- Children’s Health Insurance Program – The state of North Dakota has a great assistance program for working families with children that do not qualify for Medicaid, but still cannot afford insurance. To complete an application, you can apply online at: www.nd.gov/dhs/services/medicalserv/chip/ or contact the local Social Services office. Cavalier County’s number is (701) 256-2175. Eligibility is based on income and children’s age.
- Insurance Marketplace – The ACA has developed an online marketplace for individuals to apply for health insurance. The 2015 enrollment period has ended. However, special enrollments are still available due to life changes. Otherwise, 2016 enrollment starts November 1, 2015 and runs until January 31, 2016. You can apply online at: www.healthcare.gov.
- Charity Care – CCMH has a Charity Care policy that is based upon the Federal Poverty Income guidelines. Applicants can qualify for discounted or free care dependent upon income and liabilities. A Charity Care application has been included in this packet. Our policy and additional applications can be found on our website at: www.cavaliercountyhospital.com. The following information is required for a Charity Care determination:
a Medicaid approval/denial, the most recent tax return, and 3 months of recent paystubs or bank statements. These can be mailed or dropped off directly to the hospital or clinic locations.
